West Yorkshire ICB place Bradford District & Craven HCP selects IEG4’s All Age Continuing Care platform to Transform their services

February 2025: West Yorkshire ICB has chosen IEG4 to deploy a comprehensive digital Continuing Healthcare (CHC) platform at one of its five place-based partnerships, Bradford District and Craven Health and Care Partnership (HCP). The selection is designed to streamline the All Age Continuing Care (AACC) process throughout Bradford and enhance the department’s operational capabilities.

The innovative CHC solution, developed by IEG4 (part of the IEG Group), will enhance patient experience, improve clinical efficiency, and support better health outcomes. The platform integrates multiple processes into one digital system, improving workflows for staff and allowing the management of the entire assessment and referral process end-to-end, whilst also managing case management for patients that are eligible for AACC & S117.

The new system will fulfil the Bradford & Craven HCP’s need for a platform accessible to both staff and patients, allowing for the digitisation and automation of referral forms. It will also feature a portal that provides users with real-time updates on progress, along with 24/7 access to administrative and payment tools when necessary, offering complete end-to-end visibility of the complex process.

Previously, Bradford & Craven HCP faced challenges with its platform that demanded extensive manual input due to the absence of a digital end-to-end system.. The platform can also map data fields to meet the required fields in the Patient Level Data Set (PLDS), which enables an ICB to generate an extract of the data required for the PLDS, a monthly reporting requirement set out by NHSE, with all statutory reporting eventually done via the PLDS.
